Hi Folk

I am completely lost and hoping for a simple solution

I am currrently completing my 2021/2022 personal tax return.
I was the director of a ltd company and it was struck off a few months back.
I drew the money left out of the bank, and now I do not know how or where to declare it on my tax return. I have got to the capital gains pages, after that I dont know where to put what as there are so many boxes and pages, + i dont understand the accountancy jargon.

My accountant whom not with me anymore, said that all i need to put in is
Proceeds ( net assets) £19448
Cost (shareholding) £10
Capital gain £19438

I am stuck, however, as I dont know where to put what. Is this an 'unlisted shares' capital gain? and which boxes do i type the figures in??/ HELP
