Leaving the UK - Self assessment Q

Hi

Im leaving the UK on Saturday permanently, I've been self employed up until now my question is, do i need to submit a tax return now or after April 5th 2015?

And also what form do I need to fill in for HMRC?

All help is appreciated

P85 states:

Do not fill in this form [ie P85] if:

you normally live in the UK and are going abroad for short periods, for example on holiday or a business trip, or

you have completed, or are required to complete a Self Assessment tax return for the tax year that you leave.
