Certificate and claim to relief

I made a small investment via crowdfunding website and I've been sent a letter from HMRC that's titled Enterprise Investment Scheme Certificate and claim to relief.
Apologies if its obvious do I need to fill the form out ?

If it helps with any advice provided I have a job that I pay tax on.

Are you sure HMRC sent it to you and not the company you have invested in?

Do you want to claim any tax relief you may be due?

If not don't fill it in. If you do how else do you anticipate claiming the relief??
